ABSTRACT

Based on WANG Xugao's “thirty methods of treating the liver”, it is believed that the occurrence and development of childhood tic disorders follow the dynamic progression from liver qi disease to liver fire disease and then liver wind disease. The basic pathogenesis of three stages are characterized by binding constraint of liver qi, liver fire hyperactivity, and internal stirring of liver wind. Moreover, liver-blood deficiency and stagnation, and malnutrition of liver yin as the main point in terms of the imbalance of liver qi, blood, yin, and yang should be considered, as well as the imbalance relationship of the five zang organs such as the involvement of other organs and the gradually reach of the other organs. Guided by the principles of “thirty methods of treating the liver”, the treatment of tic disorders in liver qi stage should focus on soothing the liver and rectifying qi, soothing the liver and unblocking the collaterals, using Xiaochaihu Decoction (小柴胡汤) and Sini Powder (四逆散). The treatment of tic disorders in liver fire stage involves clearing, draining and resolving liver heat, using Longdan Xiegan Decoction (龙胆泻肝汤), Xieqing Pill (泻青丸), Danggui Longhui Pill (当归龙荟丸), and Huagan Decoction (化肝煎). The treatment of tic disorders in liver wind stage involves extinguishing wind and subduing yang, using Lingjiao Gouteng Decoction (羚角钩藤汤) and Liuwei Dihuang Pill (六味地黄丸). Throughout the treatment process, attention should be paid to harmonizing the liver's qi, blood, yin, and yang, as well as addressing the pathology of other organs.

ABSTRACT

Objetivo: realizar uma revisão integrativa a respeito da função pulmonar e da força muscular respiratória nos músicos de instrumentos de sopro. A relação da função respiratória com a utilização de instrumentos musicais de sopro é uma área do conhecimento ainda pouco explorada. Métodos: Realizada a revisão bibliográfica nas bases de dados MEDLINE, Embase, Cochrane, PeDro, BVS, Scopus, Web of Science e SciELO, através da combinação das palavras-chave "respiratory function test", "wind instrument", musician, "pulmonary ventilation" e "Lung Function Test". Resultados: Inicialmente foram encontrados 108 artigos, sendo que destes foram selecionados 11, totalizando 596 músicos instrumentistas de sopro, que fizeram parte dos grupos de estudo. Na maioria dos estudos os músicos apresentaram valores menores do volume expirado no primeiro segundo (VEF1) e da capacidade vital forçada (CVF) na espirometria que o grupo controle. No entanto, sem diferença quanto a relação VEF1/CVF. Assim como não há diferença na força muscular respiratória ou relação com doenças respiratórias. Conclusão: Os estudos atuais a respeito da consequência do instrumento de sopro em indivíduos não são capazes de evidenciar impactos positivos ou negativos na saúde respiratória desta população.


Objective: To conduct an integrative review of lung function and respiratory muscle strength in wind instrument musicians. The relationship between respiratory function and the use of wind musical instruments is an area of knowledge that has not been extensively explored. Methods: A bibliographic review was carried out in the MEDLINE, Embase, Cochrane, PeDro, BVS, Scopus, Web of Science, and SciELO databases by combining the keywords "respiratory function test", "wind instrument", musician, "pulmonary ventilation" and "Lung Function Test". Results: Initially, 108 articles were found, of which 11 were selected, totaling 596 wind instrumentalists who were part of the study groups. In most studies, musicians showed lower values of expired volume in one second (FEV1) and forced vital capacity (FVC) in spirometry than in the control group. However, there was no difference regarding the FEV1/FVC ratio, just as there was no difference in respiratory muscle strength or relationship with respiratory diseases. Conclusion: Current studies regarding the effect of wind instruments on individuals are unable to show positive or negative impacts on the respiratory health of this population.

ABSTRACT

Objective:To explore the effects of Zhenfang Baiwanzi Decoction combined with alteplase injection on serum inflammatory factors and vascular endothelial active substances in patients with acute ischemic stroke (AIS).Methods:Randomized controlled trial. A total of 87 patients with AIS admitted to Mengcheng County Hospital of Traditional Chinese Medicine between June 2019 and July 2022 were selected as the observation subjects by prospective cohort study, and they were divided into treatment group (44 cases) and control group (43 cases) according to the last number of medical records. The control group was given thrombolytic therapy with alteplase injection on the basis of routine therapy, and the treatment group was given Zhenfang Baiwanzi Decoction on the basis of the control group. Both groups were treated for 2 weeks. TCM symptoms were scored before and after treatment, and National Institutes of Health Stroke Scale (NIHSS) was used to evaluate the degree of neurological deficit, and Activity of Daily Living Scale (ADL) was applied to evaluate the quality of life. The levels of serum IL-6, CRP, TNF-α, monocyte chemoattractant protein-1 (MCP-1), VEGF, NOS and endothelin-1 (ET-1) were detected by ELISA. The adverse reactions were recorded and the clinical efficacy was evaluated.Results:The total effective rate was 93.18% (41/44) in treatment group and 76.74% (33/43) in control group ( χ2=4.62, P=0.032). After treatment, the scores of hemiparalysis, skin insensitivity, deviated mouth and eye, inhibited speech and headache and dizziness in treatment group were significantly lower than those in the control group ( t=3.38, 3.77, 2.69, 2.60, 2.36, P<0.01 or P<0.05). The NIHSS score was significantly lower than that of the control group ( t=7.53, P<0.01) while the ADL score was significantly higher than that of the control group ( t=2.99, P<0.01). After treatment, the levels of serum IL-6, CRP, TNF-α and MCP-1 in treatment group were significantly lower than those in the control group ( t=6.07, 5.70, 5.30, 3.36, P<0.01), and the levels of VEGF [(364.54±33.04)ng/L vs. (346.86±29.63)ng/L, t=2.63] was significantly lower than that of the control group ( P<0.05), NOS [(20.77±3.12) μmol/L vs. (29.46±5.36) μmol/L, t=9.27] and ET-1 [(85.41±7.09) ng/L vs. (94.11±9.38) ng/L, t=4.89] were significantly lower than those in the control group ( P<0.01). Conclusion:Zhenfang Baiwanzi Decoction combined with alteplase injection can improve the clinical symptoms, reduce levels of inflammatory factors, protect the vascular endothelial function, and enhance the efficacy of patients with AIS.

ABSTRACT

Objective:To evaluate the efficacy of Qufeng Zhiyang Decoction combined with Fuzhiqing Ointment in the treatment of chronic perianal eczema.Methods:Randomized controlled trial. A total of 122 patients with chronic perianal eczema in Meishan People's Hospital from January 2020 to May 2021 were selected as the observation objects and divided into 2 groups according to random number table method, with 61 cases in each group. The conventional group was treated with Fuzhiqing Ointment. The study group was treated with Qufeng Zhiyang Decoction. The treatment for both groups lasted for two weeks. TCM symptom score was recored before and after treatment; the degree of skin lesions was evaluated by the area and Eczema Area and Severity Index (EASI); the quality of life of patients was evaluated by the quality of life index of dermatosis (DLQI); the number of Treg and Th17 cells was measured by flow cytometry; the ratio of Th17/Treg cells was calculated; the levels of TNF-α, IL-10 and IL-6 were detected by ELISA. The adverse reactions were recorded and the clinical efficacy was evaluated. The patients were followed up for 3 months and the recurrence was recorded.Results:The total effective rate of the study group was 86.89% (53/61) and that of the conventional group was 72.13% (44/61), with statistical significance ( χ2=4.08, P=0.044). After treatment, the scores of pruritus, spurt water, erythema and burning, upset and thirsty, yellow urine and dry urine in the study group were lower than those in the conventional group ( t values were 19.64, 49.13, 19.64, 33.14, 24.61, P<0.01); EASI and DLQI scores were lower than those in the conventional group ( t values were -6.52 and -29.33, P<0.01). After treatment, Treg cells [(6.58±0.78) % vs. (5.49±0.81) %, t=7.57] in the study group was higher than that of the conventional group ( P<0.01), Th17 cells [(1.03±0.28) % vs. (1.27±0.39) %, t=-3.90] and Th17/Treg ratio [(0.15±0.07) vs. (0.23±0.05), t=7.68] were lower than those in the conventional group ( P<0.01). After treatment, the level of IL-10 in the study group was higher than that of the conventional group ( t=-16.97, P<0.01), and the levels of IL-6 and TNF-α were lower than those in the conventional group ( t values were -21.48 and -25.59, P<0.01). During treatment, the incidence of adverse reactions was 6.56% (4/61) in the study group and 9.84% (6/61) in the conventional group, without statistical significance in the incidence of adverse reactions between the two groups ( χ2=0.44, P=0.509). During the follow-up period, the recurrence rate was 15.09% (8/53) in the study group and 50.00% (22/44) in the conventional group, with statistical significance in the recurrence rate between the two groups ( χ2=14.55, P<0.01). Conclusion:Qufeng Zhiyang Decoction combined with Fuzhiqing Ointment can reduce the levels of inflammatory factors in patients with chronic perianal eczema, maintain Th17/Treg cell immune balance, improve clinical efficacy, and have a low recurrence rate.

ABSTRACT

Objective To construct a rapid prediction system to improve the accuracy and efficiency of evaluation of the consequences of nuclear accidents at a field scale. Methods Base on a diagnostic wind field model and Lagrangian particle diffusion, we established a rapid prediction method for wind field and pollutant dispersion around complex underlying surfaces within a field scale, in a way of visual discrimination of buildings and vegetation distribution. With data simulation and the use of a real urban field example, the simulated results were compared with wind tunnel test measurements and computational fluid dynamics results to study the influence of complex underlying surfaces on wind field and pollutant transport in the region. Results The rapid prediction system could clearly simulate the high-resolution wind field and pollutant concentration distribution of the region in about five minutes. It could interface with geographic information software and couple with a mesoscale weather prediction model. In terms of accuracy, the system performed well in wind field simulation, with the fractional deviations of wind speed and wind direction being 0.33 and −0.08, respectively. Concentration field simulation was greatly affected by the wind field, and the ratios of simulated concentrations to observed concentrations were between 0.05 and 3.4, except for a few low concentration points. Conclusion The rapid prediction system can effectively simulate the distribution characteristics of the flow field and improve calculation efficiency when ensuring calculation accuracy, which provides an important reference for emergency response to nuclear accidents.

ABSTRACT

This paper summarized LI Yuanwen's clinical experience in treating children with atopic dermatitis. It is considered that the core pathogenesis of children's atopic dermatitis is liver constraint and spleen deficiency, for which the main treatment methods are soothing the liver and fortifying the spleen, regulating blood and extinguishing wind, and self-made Jiawei Guomin Decoction (加味过敏煎) can be used. In clinical practice, children with atopic dermatitis are mainly divided into six types including heart fire hyperactivity, blood heat and exuberant toxin, damp pathogen congestion, disharmony of spleen and stomach, blood deficiency and wind-dryness, and yang deficiency and blood stasis. Jiawei Guomin Decoction with modifications in accordance with differentiated syndromes is recommended together with external used Qingshi Zhiyang Ointment (青石止痒软膏).

ABSTRACT

ObjectiveTo explore the mechanism of Danggui Niantongtang (DGNTT) against adjuvant-induced arthritis (AA) in rats with wind-dampness-heat arthralgia (FSR) based on the variation of intestinal flora. MethodA total of 60 SD rats were randomized into normal (control) group, FSR group, low-, medium-, and high-dose DGNTT (5.67, 11.34, 22.68 g·kg-1) groups, and methotrexate (MTX) group (1.35 mg·kg-1), with 10 rats in each group. The rats, except the control group, were injected with Mtb adjuvant and then exposed to artificial climatic chamber (hot and humid with wind) for 64 h for modeling. The rats were treated with water, DGNTT or MTX for 28 days from the day of injection. Arthritis index (AI) of rats was measured and paw volume was determined with a volume meter. The morphology of synovial tissues of the knees was observed based on hematoxylin-eosin (HE) staining and the changes of intestinal flora were analyzed based on 16S rRNA sequencing. ResultDGNTT can alleviate the hyperplasia of synovial tissue and inflammation of AA rats with FSR and inhibit the formation of pannus. The results of 16S rRNA sequencing showed that the relative abundance of Firmicutes, Lactobacillus, Prevotella 9, and Alloprevotella decreased (P<0.05, P<0.01) and the relative abundance of Bacteroidetes and Bacteroides increased (P<0.01) in FSR group compared those in the control group. Compared with the FSR group, all DGNTT groups and MTX group had high relative abundance of Lactobacillus (P<0.05, P<0.01) and low relative abundance of Bacteroidetes (P<0.01) and medium-dose and high-dose DGNTT groups and MTX group showed high abundance of Firmicutes, Prevotella 9, and Alloprevotella and low abundance of Bacteroides (P<0.05, P<0.01). Spearman's correlation analysis suggested that the abundance of Bacteroides and Helicobacter was in positive correlation with AI (P<0.05), while the abundance of Prevotella 9 and Candidatus Saccharimonas was in negative correlation with AI (P<0.01, P<0.05). There was a negative correlation between the abundance of Prevotella 9 and paw volume (P<0.01), and the abundance of Ruminococcaceae NK4A214 group, Christensenellaceae R-7 group, and Bacteroides was in negative correlation with spleen index (P<0.05). The abundance of Prevotella 9 was in negative correlation with spleen index (P<0.01). ConclusionDGNTT is effective for arthritis with FSR, as it can regulate the composition of intestinal flora in AA rats by increasing the abundance of probiotics and inhibiting the growth of pathogenic bacteria. The mechanism is the likelihood that it improves intestinal immune metabolism to ensure intestinal homeostasis.

ABSTRACT

Regan Syrup has the effect of clearing heat, releasing exterior, benefiting pharynx and relieving cough, and previous phase Ⅱ clinical trial showed that the efficacy of Regan Syrup high-dose and low-dose groups was better than that of the placebo group, and there was no statistically significant difference in the safety between the three groups. The present study was conducted to further investigate the efficacy and safety of the recommended dose(20 mL) of Regan Syrup in the treatment of common cold(wind-heat syndrome). Patients who met the inclusion and exclusion criteria were selected and divided into the test group(Regan Syrup+Shufeng Jiedu Capsules placebo), positive drug group(Regan Syrup placebo+Shufeng Jiedu Capsules) and placebo group(Regan Syrup placebo+Shufeng Jiedu Capsules placebo) at a 1∶1∶1 using a block randomization method. The course of treatment was 3 days. A total of 119 subjects were included from six study centers, 39 in the test group, 40 in the positive drug group and 40 in the placebo group. The onset time of antipyretic effect was shorter in the test group than in the placebo group(P≤0.01) and the positive drug group, but the difference between the test group and the positive drug group was not significant. The test group was superior to the positive drug group in terms of fever resolution(P<0.05), and had a shorter onset time of fever resolution than the placebo group, but without obvious difference between the two groups. Compared to the positive drug group, the test group had shortened disappearance time of all symptoms(P≤0.000 1). In addition, the test group was better than the positive drug group and the placebo group in relieving symptoms of sore throat and fever(P<0.05), and in terms of clinical efficacy, the recovery rate of common cold(wind-heat syndrome) was improved in the test group compared to that in the placebo group(P<0.05). On the fourth day after treatment, the total TCM syndrome score in both test group and positive drug group was lower than that in the placebo group(P<0.05). There was no significant difference in the incidence of adverse events between three groups and none of them experienced any serious adverse events related to the study drug. The results indicated that Regan Syrup could shorten the onset time of antipyretic effect, reduce the time of fever resolution, alleviate the symptoms such as sore throat and fever caused by wind-heat cold, reduce the total score of Chinese medicine symptoms, and improve the clinical recovery rate with good safety.

ABSTRACT

Abstract The Brazilian Caatinga has already lost extensive areas of original vegetation, thus it becomes imperative to perform fauna inventories within this region to fill geographical sampling gaps. Herein, we present a taxonomic list of the herpetofauna of a mountain chain located in the central zone of Rio Grande do Norte (RN) state whose region includes two priority areas for conservation of the Caatinga: "CA087 - Serra de Santana" in the west, and "CA078 - Nascente do Potengi" in the east. The sampling was carried out using methods of visual searching, pitfall traps with drift fences, specimens rescued during vegetation suppression activities in wind energy projects, occasional encounters, and third-party records. We recorded 19 amphibian species and 53 reptile species (23 lizards, 24 snakes, five amphisbaenians and one chelonian). About half of the recorded species have distributions entirely or mostly in the Caatinga. The mountain range sampled in this study harbors virtually all species found in nearby lowlands of the "sertaneja" depression of RN state, plus some relevant species with relictual distributions in the Caatinga, highlighting the importance of these highland areas for conservation of the Caatinga herpetofauna.


Resumo A Caatinga brasileira já perdeu extensas áreas de vegetação original, por isso torna-se importante realizar inventários de fauna nesta região para preencher lacunas geográficas de amostragem. Aqui, apresentamos uma lista taxonômica da herpetofauna de uma cadeia serrana localizada na zona central do estado do Rio Grande do Norte (RN) e cuja região inclui duas áreas prioritárias para conservação da Caatinga: "CA087 - Serra de Santana" a oeste, e "CA078 - Nascente do Potengi" a leste. A amostragem foi realizada por meio de métodos de busca visual, armadilhas de interceptação e queda, espécimes resgatados durante atividades de supressão vegetal em projetos de energia eólica, encontros ocasionais e registros de terceiros. Registramos 19 espécies de anfíbios e 53 espécies de répteis (23 lagartos, 24 serpentes, cinco anfisbênias e um quelônio). Cerca de metade das espécies registradas tem distribuição inteiramente ou predominantemente na Caatinga. A cadeia serrana amostrada neste estudo abriga praticamente todas as espécies encontradas nas planícies próximas da depressão sertaneja do RN, além de algumas espécies relevantes com distribuição relictual na Caatinga, destacando a importância dessas áreas de altitude para a conservação da herpetofauna da Caatinga.

ABSTRACT

From the perspective of academic history, the paper reviews systematically the background and evolution of the understanding of "Fengshi (GB 31) for treating wind disorders". In the ancient literature, there are no direct relevant statement for the indication of Fengshi (GB 31) associated with "wind", and the consensus on "Fengshi for treating wind disorders" has not been made yet. Under the influence of acupoint theory in recent era and the syndrome differentiation for acupuncture treatment in modern time, this statement becomes a conventional understanding and acceptable gradually. Meanwhile, the understanding for Fengshi (GB 31) treating wind disorders tends to be generalized. Practically, Fengshi (GB 31) is applicable for the various disorders in the local and adjacent areas. It is necessary for modern acupuncture researchers to systematically collate, investigate and identify the knowledge content with a sense of familiarity so that the contemporary inheritance, development and application of traditional theoretical knowledge of acupuncture can be enhanced.

ABSTRACT

ObjectiveTo investigate the clinical efficacy of Guizhi Shaoyao Zhimutang (GZSYZM) combined with fire needling in the treatment of periarthritis of shoulder with wind-cold-dampness impediment syndrome by stimulating pain points and "shoulder three acupoints". MethodA total of 120 patients with periarthritis of shoulder with wind-cold-dampness impediment syndrome, admitted to Hainan General Hospital from August 2020 to August 2022, were randomly divided into two groups using a random number table. The control group (60 cases) received treatment with GZSYZM for two weeks, while the observation group (60 cases) received treatment with GZSYZM combined with fire needling at pain points and "shoulder three acupoints" for two weeks. The clinical efficacy, adverse reactions, shoulder pain intensity, shoulder joint function, and levels of calcitonin gene-related peptide (CGRP), cyclooxygenase-2 (COX-2), interleukin (IL)-2, and IL-17 before and after treatment were compared between the two groups. ResultThe total effective rate in the observation group was 88.33% (53/60), significantly higher than 68.33% (41/60) of the control group (χ²=7.070, P<0.01). Compared with the results before treatment, both groups showed significant reductions in pain rating index (PRI), visual analog scale (VAS) scores, present pain intensity (PPI), and Simplified McGill Pain Questionnaire (SF-MPQ) total scores, as well as serum levels of CGRP, COX-2, IL-2, and IL-17 after treatment (P<0.01), and improved pain intensity, daily life abilities, joint mobility, muscle strength, and Constant-Murley score (P<0.01). Compared with the control group after treatment, the observation group showed significantly reduced PRI, VAS score, PPI, SF-MPQ total score, as well as serum levels of CGRP, COX-2, IL-2, and IL-17 (P<0.01), and increased pain intensity, daily life abilities, joint mobility, muscle strength, and Constant-Murley score (P<0.01). There was no significant difference in the incidence of adverse reactions between the two groups. ConclusionGZSYZM combined with fire needling at pain points and "shoulder three acupoints" can effectively reduce the levels of serum inflammatory factors and pain mediators, alleviate pain, and improve shoulder joint function in patients with periarthritis of shoulder with wind-cold-dampness impediment syndrome.

ABSTRACT

Since LIU Hejian proposed the concept of sweat pore, the theory of sweat pore has experienced accelerated development. Especially with the advances in modern human anatomy and physiology, the microscopic anatomy of sweat pore begins to focus on the intercellular space, ion channels and other membranous space with channels, pores, doors, etc., which exert the functions of exchanging fluid, information, and energy inside and outside blood vessels and discharging metabolic wastes so as to maintain the normal operation of organs. Therefore, sweat pore is the structural basis for the movement of Qi and the central link of Qi-fluid exchange in the body. The brain, as the house of original spirit, is in charge of the spirit of five Zang-organs. The brain sweat pore is pivotal for the circulation of Qi, blood, and fluid in the brain, and it is the structural basis for the normal physiological functions of the brain. The dysfunction of the brain sweat pore will cause the stagnation of Qi and the abnormal transport of blood and fluid. It will cause the abnormal exchange of Qi, liquid and other material and information, which fail to nourish the original spirt and cause the loss of vital activity, eventually leading to consciousness and emotion disorders. The treatment should focus on opening the brain sweat pore, smoothing the exchange of Qi and fluid inside and outside the pore, and restoring the Qi movement, so as to cure encephalopathy. At present, western medicine treatment of encephalopathy needs to solve the problem of drug efflux from the blood-brain barrier and improve the effective concentration of drugs into the brain. The structure and function of brain sweat pore is similar to those of the blood-brain barrier. The aromatic resuscitative medicines and wind-extinguishing medicines can open the brain sweat pore. When being combined with other medicines, they can lead the medicine to enter the brain to restore the Qi movement of the brain sweat pore and enhance the therapeutic effect. Liver-pacifying wind-extinguishing medicines, insect medicines, tonifying medicines, heat-clearing toxin-removing medicines, and damp-draining medicines can treat pathological factors such as wind, phlegm, stasis, deficiency, toxin, and dampness, respectively. These medicines, combined with the medicines with the tropism to brain meridians, can open the brain sweat pore and guide the medicine into the brain to enhance the effective concentration of the medicine, thereby enhancing the efficacy against encephalopathy.

ABSTRACT

ObjectiveTo observe the clinical effect of modified Yinqiaosan combined with antibiotics in the treatment of acute tonsillitis in children with wind-heat invading lung syndrome. MethodA total of 96 children with acute tonsillitis of wind-heat invading the lung syndrome were randomized into control group and observation group. The control group was treated with routine antibiotics, and the observation group was treated with modified Yinqiaosan and antibiotics for 7 days. The scores of major symptoms (sore throat, erythmatous throat, dysphagia, erythmatous tonsils and suppuration) and minor symptoms (fever, cough, stool, and tongue) and the levels of inflammation- and immune-related indexes [white blood cell (WBC), C-reactive protein (CRP), serum amyloid A (SAA), interleukin-6 (IL-6), and interferon-γ (IFN-γ)] were compared between two groups. ResultThe data of 92 children were statistically analyzed: 45 in the observation group and 47 cases in the control group. The total clinical effective rate of the observation group was 95.56%, as compared with the 93.62% of the control group. After treatment, the scores of major symptoms in the observation group were lower than those in the control group (P<0.05), and the scores of cough, defecation, and tongue in the observation group were lower than those in the control group (P<0.05). The levels of inflammation- and immune-related indexes after treatment in the observation group were lower than those before treatment (P<0.05). Except IFN-γ, the levels of the inflammation- and immune-related indexes in the control group were lower than those before treatment (P<0.05). After treatment, the levels of SAA and IL-6 in the observation group were lower than those in the control group (P<0.05). ConclusionModified Yinqiaosan combined with antibiotics can significantly reduce the expression of SAA and IL-6 in the treatment of children with acute tonsillitis, thereby alleviating inflammation and clinical symptoms and improving prognosis.

A geomagnetic storms is a global disturbance in Earth's magnetic field usually occurred due to abnormal conditions in the interplanetary magnetic field (IMF) & solar wind plasma emissions caused by various solar phenomenon. 138 solar wind streams in solar cycle 22 & 173 solar wind streams in solar cycle 23, have been found, which are associated with proton density, observed during 1986 to 2010. The wind, phlegm, and blood stasis are important pathogenic factors of cough variant asthma in children, and they are also the pathological products in the occurrence and development of this disease. They have typical pathogenic characteristics. The main pathogenesis characteristics of cough caused by wind, phlegm and blood stasis are as follows that external wind attacks the lungs and induces internal wind, phlegm and dampness accumulates in the lungs, and the lungs fail to declare and descend, and blood stasis obstructs the collaterals and stagnation of Qi. The wind, phlegm, and blood stasis have their own pathogenic characteristics, and their cough-causing also have their own pathogenic characteristics and clinical characteristics. Based on the characteristics of wind, phlegm, and blood stasis, the application of medicine based on the differentiation of symptoms and signs can effectively prevent and treat this disease, and provide theoretical basis and treatment ideas for the treatment of cough variant asthma in children with Chinese medicine.

ABSTRACT

The application of traditional Chinese medicines (TCMs) has a history of more than 2000 years, which have the characteristics of multi-component, multi-target, and high safety. Post-infectious cough (PIC) is a respiratory disease with high incidence. It belongs to subacute cough and accounts for as much as 40%–50%. Cough is the main clinical manifestation of PIC. PIC seriously affects people's life quality because of complex etiology, long-term course of disease, treatment difficulties and other characteristics. Western medicines are based on the principle of symptomatic treatment, so they are often difficult to control PIC fundamentally. These factors could due to that PIC is prolonged and unable to heal repeatedly. TCMs have obvious advantages in treating PIC, with accurate curative effects, less side effects and adverse reactions and are effective in improving PIC-related symptoms and indicators, enhancing patients' life quality and reducing pain. TCMs, guided by holistic concept and syndrome differentiation, advocate determine treatment on the basis of pattern types, and have remarkable clinical treatment effects. As for TCMs etiology, pathogenesis and syndrome types of PIC, TCM scholars have not yet reached a unified standard. However, most of them think that wind pathogen can cause PIC alone, or it can be combined with other evils, which might be the main mechanism of PIC. This paper discusses the advantages and limitations of TCMs in PIC treatment from etiology, pathogenesis, distribution of syndrome types and treatment of TCMs. This article focuses on the treatment methods and pharmacodynamic material basis of wind pathogen, providing ideas in treating PIC of TCMs clinically and innovative drug development.

ABSTRACT

Objective:To evaluate the clinical efficacy of self-made Tingchan Decoction combined with dopasizin tablets in the treatment of Parkinson disease (PD) in the middle and early stages of yin deficiency and wind movement syndrome.Methods:From January 2020 to March 2021, 84 patients with early-stage PD in Weifang Hospital of Traditional Chinese Medicine in Shandong Province who met the inclusion criteria were divided into 2 groups according to the random number table method, with 42 in each group. The control group was given oral dopasehydrazine tablets, and the observation group and the control group were additionally given the self-made Tingchan Decoction. Both groups were treated for 8 weeks. TCM syndrome scores were performed before and after treatment, and the Unified Parkinson Disease Rating Scale (UPDRS) was used to evaluate the severity of PD. The ELISA was used to determine serum homocysteine (Hcy), high mobility the levels of group protein-1 (HMGB1), IL-6, IL-2, substance P and dopamine (DA). The levels of glutamate (Glu) and γ-aminobutyric acid (GABA) were detected by HPLC. The OD value was determined by photometer, and the relative expression levels of microRNA-124 (miR-124) and microRNA-425 (miR-425) were calculated by the 2 -??Ct method to evaluate the clinical efficacy. Results:The total effective rate was 88.1% (37/42) in the observation group and 73.8% (31/42) in the control group, and the difference between the two groups was statistically significant ( Z=-2.56, P=0.011). after treatment, the action score, tremor score, upper limb coordination score and gait score in the observation group were significantly lower than those in the control group ( t values were 7.23, 5.80, 4.25, 4.17, 15.00,respectively, all Ps<0.01). After treatment, the levels of serum Hcy, HMGB1, IL-6 and IL-2 in the observation group were significantly lower than those in the control group ( t values were 12.16, 10.67, 23.11, 9.95, respectively, all Ps<0.01), serum Glu [(71.28±6.46) μmol/L vs. (56.91±5.87) μmol/L, t=10.67], GABA [(292.39±15.46) μmol/L vs. (248.51±14.38) μmol/L, t=13.47] levels in the observation group were significantly higher than those in he control group ( P<0.01); the level of serum substance P [(3.54±0.43) mg/L vs. (5.61±0.52) mg/L, t=19.88] was significantly lower than that of the control group ( P<0.01). The DA [(79.24±0.52) ng/L vs. (70.15±5.36) ng/L, t=7.93] and miR-124 [(4.57±0.74) vs. (2.81±0.47), t=13.01], miR-425 [(3.94±0.83) vs. (2.73±0.97), t=6.14] expression in the observation group were significantly higher than those in the control group ( P<0.01). Conclusion:Self-made Tingchan Decoction combined with dobasilazine can increase the relative expression of miR-124 and miR-425 in the serum of PD patients, promote the release of dopamine, reduce the oxidative stress in the brain, reduce the level of serum inflammatory cytokines, improve the Symptoms, and protect nerve cells.

ABSTRACT

ObjectiveTo explore the mechanism of Danggui Niantongtang (DGNT) against adjuvant arthritis (AA) rats with wind-dampness-heat arthralgia by quantitative proteomics. MethodSixty SD rats were randomly divided into normal group, model group, angelica came pain soup low, medium and high dose group and methotrexate (MTX) group, each group of 10, only the rat tail root subcutaneously inactivated mycobacterium tuberculosis (Mtb) of adjuvant to build model of AA, artificial climate box intervention 16 d rheumatic fever bi syndrome model is set up, building the day began to drug intervention, The intervention lasted for 28 days. The proteins of synovial tissues in experimental rats were extracted. The differential proteins in the medium-dose DGNT group and the model group were detected and analyzed by 4D label-free quantification (4D-LFQ) proteomics. The differentially expressed proteins associated with mitochondrial pathway apoptosis were verified by immunohistochemistry and Western blot. ResultA total of 4 756 proteins were identified from rat synovial tissues, of which 4 234 proteins contained quantitative information. There were 814 differential proteins between the model group and the DGNT group. As revealed by Gene Ontology (GO) and Kyoto Encyclopedia of Gene and Genome (KEGG) enrichment analyses, DGNT had an effect on the synovial proteome of AA rats with wind-dampness-heat arthralgia, and the differential proteins were enriched in the regulation of the immune system, response to acute inflammation, and apoptosis regulation. As demonstrated by the results of immunohistochemistry and Western blot, compared with the model group, the DGNT groups and the MTX group showed increased protein expression of B-cell lymphoma 2 (Bcl-2)-associated X protein (Bax) and cytochrome C (Cyt C)(P<0.05, P<0.01), reduced Bcl-2 level (P<0.05, P<0.01), elevated level of cleaved cysteinyl aspartate-specific protease 9 (Caspase-9)/Caspase-9 (P<0.01), and decreased level of phosphorylated protein kinase B (p-Akt)/Akt(P<0.05, P<0.01). ConclusionDGNT involved multiple targets in the treatment of AA with wind-dampness-heat arthralgia and it may exert its effect in the prevention and treatment by regulating the Akt/Bax/Bcl-2 pathway and promoting the cell apoptosis in the mitochondrial pathway.

Hypertension is the most important risk factor for cardiovascular and cerebrovascular diseases, affecting the structures and functions of important organs of the body, such as the heart, brain, and kidney. At present, the prevalence of hypertension in China remains high. How to effectively curb the incidence of hypertension and reduce target organ damage in patients with hypertension is an urgent challenge that needs to be addressed. Traditional Chinese medicine (TCM), by virtue of its unique efficacies and advantages, is increasingly applied around the world. In TCM, "wind" is considered as a major constant factor in the development of hypertension. Some scholars believe that hypertension is located in collaterals, and the lesions of collaterals are also important reasons for the occurrence and development of hypertension. Collateral diseases and pathogenic wind are closely related to the development of hypertension as well as target organ damage in the heart, brain, and kidney. From the six-meridian syndrome differentiation for febrile diseases, "collateral" and "wind" are closely related to the Jueyin, with collateral diseases classified into Jueyin diseases and pathogenic wind beginning in the Jueyin. The occurrence, development, and persistence of hypertension are closely related to Jueyin diseases. The present study analyzed the pathogenesis and treatment of hypertension from "collateral" and "wind" of the Jueyin, and specifically discussed the relationship between hypertension and "collateral" and "wind" of the Jueyin. It is believed that the internal depression of wind and fire in the Jueyin results in the upward impulse of liver fire, and the deficiency of Jueyin can trigger the internal movement of liver wind and stirring wind due to collateral deficiency. External contraction in the Jueyin due to the induction of external wind is the important pathogenesis of the development of hypertension and the damage to target organs such as the heart, brain, and kidney. The therapeutic methods for both "collateral" and "wind" were also discussed based on the primary prescription for Jueyin, Wumei Pills. Six-meridian syndrome differentiation can guide the therapeutic principles for all diseases and inspire posterity. Exploring the pathogenesis and treatment of hypertension from “collateral" and "wind" of the Jueyin is of great significance in guiding the prevention and treatment of hypertension, reducing target organ damage in patients with hypertension, and improving the prognosis of cardiovascular and cerebrovascular diseases.
